Pet Grooming Consent

• I am the rightful owner of the dog(s) for which the services are being rendered.


• I confirm my pet(s) is healthy, up to date on vaccinations, de-wormed and de-flead.


• I will not hold groomer responsible or liable in the event grooming reveals a preexisting condition which may need to be treated before grooming can commence.


• I understand the groomer will do their best to follow my requests and/or instructions, but in situations especially where it is interfering with the pet’s comfort and/or making it difficult to groom without injuring the pet may need to be cut or shave than was requested e.g. because of matting or knots. In this case, groomer would call to get permission before any shaving is done.


• I am aware that shaving a heavily matted coat can cause nicks, cuts or abrasions due to skin growths trapped in the mats. Heavy matting can trap moisture and urine near the pet’s skin allowing mold, fungus or bacteria to grow causing skin infections that existed prior to grooming process.


• I understand if fleas and tick infestations are found on my pet, the groomer will need to give a medicated bath at my expense.


• I understand a muzzle may be used if necessary. Muzzling protects both the pet and groomer and can often calm stressed animals.


• I understand having a pet grooming carries some risk. Although the utmost care is taken to prevent injury, grooming instruments can be sharp and accidents can occur such as nicks, cuts, scratches, piercing etc. {However, any unintentional or untoward incident must be informed, no matter how small, and if necessary, I authorise the groomer to transport my pet to the nearest vet for further care.} Veterinary bills caused by serious matting, preexisting conditions, aggressive or difficult temperament or from the pet being elderly, are my responsibility.


• I am aware that mat removal procedures can cause itchiness, skin redness, self-inflicted irritations or abrasions and failure for the hair to grow.


• I am aware that any grooming adjustments have to be informed immediately after the groom {or before the pick up of my pet(s).} Any return visits will be treated as new appointment and charged a grooming fee.


• Estimates are made on an average of dog breed or type, and I am ready to pay additional if my pet requires additional time and extensive work.


• I agree to my pet’s photograph being taken and it being used for marketing purposes.

I confirm that have read fully and understand the information in this consent form and all details included. I agree to accept all and fully responsible for any risks, injuries, damages or accidents which may impact my pet. I will not hold groomer responsible for any conditions present, but not disclosed at the time of grooming that may affect grooming.
